Dear Parents and Guardians:

Health Sciences Charter School has partnered with the Erie County Department of Health to offer voluntary COVID-19 testing options for those who may be interested in having this service available for their children. This free and voluntary program is in collaboration with Quadrant BioSciences, Inc.  School districts in New York State must have the capacity to offer this service pursuant to 10 NYCRR 2.62.  Families can volunteer their children for surveillance and/or proximal testing.  We anticipate that this program will begin in October 2021.

Surveillance Testing:

  • This is a random selection of 10% of the building population who have consented to participate on a weekly basis. Students will participate in the screening during their school day.

Proximal Testing:

  • This is available if/when your child is exposed to someone at school who recently tested positive for COVID-19, but your child does not meet “close contact” status and is not subject to quarantine.
  • You will be notified of the test result via If your child has a positive COVID-19 test result, a contact tracer will call you to place your child in quarantine.

PLEASE NOTE: Both tests involve a swab inside the mouth or a minimally invasive nasal swab that the student themselves will perform. Your child should not eat or drink anything within 30 minutes of the scheduled test.

Program Registration:

If, after you have volunteered you no longer wish for your child to participate in the surveillance testing program, please visit to rescind your consent.
